Patient Center/Referrals

An introduction to our office policies; 

All new patients:

  • Please arrive 15 minutes prior to your appointment to complete your registration.
  • You may print a registration form  to complete by selecting the "Patient Registration Form" tab on this page. Please complete the form and bring it with you for your appointment.
  • Be sure to also bring your health insurance card/s and referral, to your appointment. 

Pulmonary Consults:

  • Please bring any recent chest x-rays or CT scans (films and reports), recent lab work, a list of your medications, and recent office notes from your referring physician.
  • The referring physician's office may fax your information to (202) 835-9040.
  • Please ask them to indicate which physician and the date and time of your appointment.
  • Allow yourself 90 minutes for this appointment.


  • Nothing to eat or drink after midnight the night before your appointment. You may have water.
  • If you have medications you take each morning, you may take them the morning of your physical with water.
  • Please do not apply any powder, lotion, or cream to your chest, legs, or arms on appointment day.
  • Allow yourself 60 minutes for this appointment.

Wait Time:  

We sincerely attempt to see each patient at the scheduled appointment time and we do not "double book" patient visits.  We understand your time is valuable and excessive wait time is stressful. However, pulmonologists routinely see new or established patients with emergencies or urgent problems. We will attempt to communicate any delay and alternatives as we become aware of them.  Thank you for your patience.

Contacting our office:

  • Our appointment hours are from 8:00 am to 11:45 am and 1:45 pm 3:45 pm, each physician's starting appointment times are slightly different.
  • Our phone lines are answered from 9:00 am till 12:00 noon and 1:00 pm till 4:00 pm.
  • If you have an emergency when the office is closed, our answering service will be able to contact one of the physicians.
  • We will make every effort to return non-emergency calls within 24 hours.
  • Please keep in mind when we are assisting scheduled appointments, we will return your call as soon as possible. 

Prescription Refills:

  • We encourage you to obtain all your refills while you are in the office. Please review your medications and bring a list of refills you require.
  • When you have no less than one week of medication remaining, please contact your pharmacy and ask them to fax a refill request to our office.  
  • For your safety we refill prescriptions electronically.
  • If your medication requires a prior authorization, please contact your insurance and have them fax a prior authorization form to our office. 
  • Refills cannot be handled after regular office hours.


For patients referred to our office by their primary care physician as consults, it is your responsibility to make sure you have a valid referral for each visit, if required by your insurance carrier. Please check that the expiration date has not lapsed and there are unused "authorized visits remaining.
